This includes removing barriers to adherence, closing gaps in care, and resolving drug therapy problems.
We also help members with preventative care and lifestyle modifications to improve their health through coaching with a dietitian. Our integrated remote monitoring devices (RPM) enhance this strategy by allowing our pharmacists to better understand the member’s condition, and outreach when appropriate. We only provide devices to members who will benefit, reducing plan waste. Our pharmacists also coordinate their care recommendations with the member’s treating physicians.
